Application for 2010/2011 Season

To register for one of our programs:

1. Fill in form online, before you press the submit button print the form and sign it. Then press Submit to show your intent to participate. Space is limited so please do this ASAP.

2. Provide a NON-REFUNDABLE deposit CHECK MADE OUT TO NEWHL, in the amount of $150 to hold your spot for the upcoming season. The remaining balance can be split into 2 payments. 1st payment is 75% of your remaining balance, it is due on the 1st day of your season. The 2nd payment (remaining balance) will be due on January 7, 2011.

3. Mail the signed registration form and deposit check to: Kerri Macaluso, 11 Putnam Rd., Acton, MA. 01720.

4. Please bring your 75% payment along with your signed application (if you haven't provided it already) to your first skills session. Please make your check out to: NEWHL. We will send out a reminder email in early September.

5. Prior to September 1, you must register online for USA Hockey membership and pay its fee directly to USA Hockey. The 2010/2011 fee is $30. Go to http://www.usahockey.com and click on Online Member Registration. You will receive a confirmation number in your email, which you must forward to Sonya@ospf.org so she can register you into the NEWHL program, which is a local affiliate of USA Hockey.  

6. Refund policy. C/D and D/Recreational levels: NO REFUNDS issued unless you find someone with equal experience to take your place with in the first month of play. Learn to Play: NO REFUNDS issued unless you find someone to take your place and it is within the first 2 weeks of the program. Our Refund Policy takes into account the safety and experience of the player and is based upon the fact that we buy the ice and make schedules based upon number of applicants. No matter what happens, we have to pay for it.

Note that full hockey equipment is required for all programs.
